Preschool Enrollment

In order to enroll a student for kindergarten, the child should be three years of age by September 1. Along with the registration form, we are requesting the following:

• A copy of one of the following: (Please provide a copy within 30 days)
  • Certified birth certificate
  • A reliable proof of child’s identity and age, such as a baptismal record
  • A social security application with registering child’s name
  • A notarized affidavit explaining the lack of birth certificate
